Terms of Service
Effective October 19, 2026
In short: we order public corporate documents from the Ontario Business Registry for you and email them to you. You confirm the corporation before paying, and we order exactly that one. If we can't deliver, you get your money back.
These terms are the full agreement. Please read them.
1. Who we are
CorpReport (corpreport.ca, the "Service") is operated by forMaD Health Inc., an Ontario corporation ("we", "us"). You can reach us at [email protected]. By placing an order you agree to these Terms. If you order on behalf of a corporation or organization, you confirm you're authorized to do so and these Terms bind it as well.
2. What the Service does
On your request, we order official search products, currently the Corporation Profile Report and the Certificate of Status, from the Ontario Business Registry operated by the Government of Ontario ("the Registry"), and deliver the document to you electronically.
- We are independent. CorpReport is not affiliated with, endorsed by, or acting for the Government of Ontario, ServiceOntario, the College of Physicians and Surgeons of Ontario (CPSO), the Royal College of Dental Surgeons of Ontario (RCDSO), or any other regulator.
- You can do this yourself. These documents can be ordered directly from the Registry for the government fee. Our price includes that fee plus our service charge.
- Public records. These documents are public records. No Company Key, registry account or authority from the corporation is needed to order them.
3. Your responsibilities
- You confirm the correct corporation before paying. For single orders you select the corporation from the Registry's own records. For bulk orders you supply the Ontario Corporation Numbers and confirm they are correct.
- You give a valid email address where the documents can be delivered.
- You use the Service lawfully and do not use it to harass, stalk or harm anyone, to resell automated access to the Registry, or to interfere with the Service or the Registry.
- You are responsible for checking what your College, bank, lender or other recipient requires. We describe common requirements on our site to help, but those organizations set and may change their own rules.
4. Prices and payment
- Prices are in Canadian dollars and include the Registry's government fee. The price is shown before you pay.
- Payment is processed by Stripe. We never see or store your full card number.
- Applicable sales taxes, if any, are shown at checkout.
- Promotional codes apply only as stated and cannot be exchanged for cash.
5. Delivery
- Most documents are delivered within minutes of payment. If the Registry is slow or unavailable, a person completes your order, usually within a few business hours. Bulk orders are processed one document at a time and may take longer.
- Documents are delivered by email and through a private link to your order page. Download links expire; ask us if you need a new one.
- We deliver documents exactly as the Registry issues them. We do not alter their content.
6. Refunds
- Not delivered: if we cannot deliver your document, we refund you in full.
- Our mistake: if we order the wrong corporation or the wrong document because of our error, we replace it at no charge or refund you in full, at your choice.
- After delivery: once the Registry has issued the document you ordered, the government fee has been spent and the order is final. This includes documents ordered for a corporation number you supplied that turns out to be incorrect.
- To request a refund, email [email protected] within 30 days of your order.
7. Accuracy of the information
The content of each document comes from the Registry. We are not responsible for errors, omissions or out-of-date information in the Registry's records, or for decisions a College, institution or other party makes about your document. The Service and the information on our site are general information only and are not legal, accounting or professional advice.
8. Reminders and emails
If you ask us to, we'll email you before your next renewal. Every reminder includes a way to stop them. We also send emails needed to deliver your order. We do not send marketing email without your consent.
9. Availability
The Service depends on the Registry and on third-party providers. We may change, suspend or discontinue any part of the Service, and we may decline or cancel an order (with a full refund) at our discretion, for example if the Registry changes its process or we suspect misuse.
10. Limitation of liability
To the extent permitted by law: the Service is provided "as is"; we are not liable for indirect, incidental, special or consequential losses, including late renewals, lost income or penalties; and our total liability for any claim relating to an order is limited to the amount you paid for that order. Nothing in these Terms limits rights you have under consumer protection law that cannot be waived, including under Ontario's Consumer Protection Act.
11. Indemnity
If you use the Service in breach of these Terms or the law and that causes a claim against us, you agree to cover our reasonable losses and costs arising from it.
12. Privacy
How we handle personal information is described in our Privacy Policy, which forms part of these Terms.
13. Changes
We may update these Terms. The version in effect when you place an order applies to that order. We'll post the new effective date at the top of this page.
14. Governing law
These Terms are governed by the laws of Ontario and the federal laws of Canada that apply there. Disputes will be heard in the courts of Ontario, except where consumer protection law gives you the right to another forum.
